Adolescence – Optimising the Permanent Dentition

Adolescence is the classic window for orthodontic treatment — and for good reason. By approximately age 12–13, the permanent dentition is complete (with the exception of wisdom teeth), the jaws are still growing and responsive to orthodontic forces, and the social and psychological motivation for treatment is often high.

The Orthodontic Assessment

Before any orthodontic treatment begins, a comprehensive assessment is performed. This includes:

  • Clinical examination of the teeth and bite
  • Study models or digital scans of the dentition
  • Orthodontic X-rays (panoramic and cephalometric)
  • Photographs of the face and dentition
  • Discussion of treatment goals with the patient and parents

Based on this assessment, the clinician determines whether treatment is indicated, what approach is most suitable, and what outcome can be realistically achieved.

Fixed Braces vs Clear Aligners

Both fixed braces and clear aligners are highly effective during adolescence. The choice depends on:

Case complexity: Fixed braces can address a wider range of tooth movements and are the preferred option for more complex cases.

Patient preference: Many teenagers prefer the discreet appearance of clear aligners. However, aligners require disciplined wear (20–22 hours per day) — compliance that not all teenagers consistently achieve.

Lifestyle considerations: Teenagers involved in contact sports may prefer the convenience of removable aligners. Musicians who play wind instruments may need special adaptations.

Life After Braces

Retention is a permanent part of orthodontic treatment. Once the active treatment phase is complete, retainers must be worn to prevent the teeth from shifting back.
