When Can Aesthetic Fillings Be Used
Tooth-coloured composite resin fillings offer an excellent combination of aesthetics and functionality for a wide range of clinical situations. Understanding when they are the appropriate choice — and when a different restoration type would serve the patient better — is key to achieving lasting results.
Indications for Composite Fillings
Small to medium cavities: Composite is ideal for restoring cavities that are not too large. In small to medium cavities on any tooth surface, composite delivers excellent aesthetics and adequate durability.
Class V lesions: Erosion or abrasion lesions at the gum line, often seen in patients who brush too hard, are well restored with composite due to the material's adhesive properties and ability to match tooth colour precisely.
Fractured or chipped teeth: Small chips or fractures — particularly in front teeth — can be repaired beautifully with composite, often in a single appointment without any tooth preparation.
Diastema closure: The space between upper front teeth (a diastema) can be closed cosmetically using composite additions to both teeth, without any grinding.
Replacing old amalgam fillings: When existing amalgam fillings require replacement (due to secondary decay, fracture, or aesthetic preference), composite is the modern material of choice.
Limitations of Composite
Composite is not the ideal solution in all situations. Large cavities involving multiple cusps, or heavily loaded areas in the posterior teeth, may be better served by an inlay, onlay, or crown. The clinician's decision is based on the size of the cavity, the remaining tooth structure, the patient's biting forces, and aesthetic requirements.
Composite also requires meticulous moisture control during placement — a demanding technique that requires an experienced clinician to execute correctly.
