The Importance of Wisdom Tooth Removal

Wisdom teeth — the third molars — are the last teeth to erupt, typically between the ages of 17 and 25. In most modern jaws, there simply is not enough space for them to emerge correctly. This makes them one of the most frequently treated problem areas in oral surgery.

Why Do Wisdom Teeth Cause Problems?

The human jaw has become progressively smaller over thousands of years of evolution, yet the number of teeth has remained the same. The result is that wisdom teeth frequently become impacted — blocked by the second molar, the jawbone, or the overlying gum tissue. Even when they partially erupt, they are extremely difficult to clean effectively and become prone to decay and gum disease.

The Most Common Complications

Pericoronitis: Partial eruption leaves a flap of gum tissue over the tooth that traps bacteria and food, leading to recurrent painful infections.

Crowding: Erupting wisdom teeth can exert pressure on the existing dentition, displacing teeth that were previously straight. This is particularly relevant in patients who have undergone orthodontic treatment.

Decay in adjacent teeth: An impacted wisdom tooth that presses against the second molar creates a space impossible to clean, making both teeth vulnerable to cavities.

Cyst formation: The sac surrounding an unerupted wisdom tooth can develop into a cyst, destroying bone and adjacent teeth if left untreated.

When Is Removal Recommended?

Not every wisdom tooth needs to be removed. Fully erupted wisdom teeth that are properly positioned and easy to clean may be retained. However, removal is recommended when the tooth is impacted, has caused repeated infections, is contributing to crowding, shows decay, or when a cyst has formed.

The ideal time for removal is in the late teens or early twenties, when the roots are not yet fully formed and the bone is more flexible. Recovery is faster and complications are fewer at this age.
